Baptismal Information Request Form

This form is for children receiving the sacraments of First Reconciliation, Confirmation, or First Eucharist that need to submit proof of baptism. This is not a form to request your child to be baptized.

In order for a child to receive First Reconciliation, Confirmation, and First Eucharist, they must be baptized. The baptismal certificate provides formal recognition of Baptism.

  If your child was baptized at St Joseph's you still need to complete this part of the form.
  
  If your child was baptized at a different parish in addition to this form you need to either turn in a copy of his or her original baptism certificate or call the parish of baptism and have them mail you a proof of baptism form to turn in.
